First Baptist Church (FBC) members/attendees will use this form to request interpretation for church events/ministries other than Sunday morning worship or church committee meetings. It is our goal to be able to provide interpreters as much as possible. Requests do not need to be submitted for Sunday morning worship nor committee meetings where a committee member requires interpretation, those will be taken care of already. All requests for interpreters outside of those events above are required to originate from filling out this form below. Requests need to be submitted 48 hours in advance of event and received during FBC’s normal business hours of Monday-Thursday 8:30am-5:00pm. Last minute requests or emergencies will be done on a case by case basis. Those needing interpretation must originate the request from this form to be submitted to FBC, not directly to interpreters or interpreting agencies. Filling out this form is the only approved church policy for requesting an interpreter. Confirmation will be sent that the form has been submitted, as well as whether an interpreter will be able to provided. This form be used for events like VBS, Summerama, Women’s Ministry Events etc. It is the strong desire of FBC to be able to provide an interpreter for all ministry events possible. Filling out of this form is not a guarantee than interpreter will be provided, it is a request. FBC must abide by approved budgets to ensure financial oversight. Interpreters may not always be available from interpreting providers as well. Specific interpreter requests may be submitted, and will be honored if possible by FBC and interpreting agency.
